Annual Picnic

Fox Run Park

Our Annual Picnic is an open community event held at Fox Run Park. All USTC students, families, friends, and guests are invited to join us for bulgogi (Korean BBQ), a soccer tournament, and a fun day of play. Our goal is to encourage a sense of community within our schools and just have fun. You can invite as many friends and family you would like to our event – it’s free and open to the public!

The deadline to register on Thu 8/8/24 is in place to the order for food. USTC always plans a little extra food. Therefore, if you did not sign up it is not too late.
This form will close Fri 8/9/24 at 7:00 PM. If you do not RSVP by that time you can still attend but plan to arrive at least 10 minutes early to get your name added to the list at the USTC tent.

LIABILITY WAIVER

This Agreement executed the date shown by and between U.S. Taekwondo Center, Inc. hereinafter referred to as the Center, and the Student, who has caused his/her signature to be affixed hereto, hereinafter referred as Enrollee.

Enrollee agrees that all exercises and/or courses are undertaken at student’s own risk. The enrollee understands the procedures and exercises involved in instruction and participation as explained to him/her by an assistant of the Center. The Enrollee understands that there is a risk of personal injury involved in the course of instruction and with this knowledge agrees to indemnify and save harmless the Center from all losses caused by accident or injury to the Enrollee, or to third persons, who may be with the Enrollee of the Center, in the event that either the enrollee or said third person is injured in any way during the performance and execution of exercises.

THEREFORE, I do hereby for myself/ my heirs/ executors/ administrators/ assigns, waives, release and forever discharge any and all rights/ claims for damages which I may sustain in connection with any practice, class, contest, athletic event, or traveling to/ from such endeavors.

The U.S. Taekwondo Center respects your privacy and will never sell or distribute your personal information to any third parties. You may be contacted by the U.S. Taekwondo Center as a follow up to your participation in this lesson.
